I.FUNCTIONAL STATEMENT



To effect the implementation of civil registration of all vital and registrable documents in the City per guidelines of the Office of the City Civil Registrar General, in coordination with the City's plans for its implementation.

To develop manpower and facilities to maintain / improve / strengthen service delivery, with a continuous search for innovative schemes to upgrade same, tapping linkages whenever needed.

A.Registration of Birth Certificate

A.1. Timely Registration
-Duly accomplished Certificate of Live Birth (M.F. Form  102)

A.2.  Late Registration
-  Duly accomplished Certificate of Live Birth (M.F. Form 102) 
-  Any of the following:
a.)    Baptismal Certificate
b.)    Form 137
c.)    Voter’s record
d.)    Income Tax return
e.)    Service Record
f.)    Driver’s License
g.)    Passport
-    and Community Tax Certificate (CEDULA) of applicant


B.Registration of Death Certificate


B.1.Timely Registration
-Duly accomplished Certificate of Death (MF Form 103)
-Barangay Death Certification notarized by the lawyer
( required in case the place of birth  did not occur in the hospital)

B.2.Late Registration
-Duly accomplished Certificate of Death (M.F. Form 103)
-Barangay Death Certification notarized by the lawyer
( required in case the place of birth  occurs not in the hospital)
-Community Tax Certificate
-Joint Affidavit  of two Disinterested persons


C.Registration of Marriage Certificate

C.1Timely Registration

-Duly accomplished Certificate of Marriage (M.F. Form 97)
-Marriage License
C.1.b.  Article 34 (Husband and Wife Living together for more than 5 yrs.)
-Duly accomplished Certificate of Marriage (M..F. Form 97)
-Joint Affidavit of Husband and Wife

C.2.Late Registration
-Duly accomplished Certificate of Marriage (M.F. Form 97)
-Negative Certification of Marriage from National Statistics Office (CENOMAR)
-Negative certification from Local Civil Registrar’s Office
-Affidavit of affiant attesting the facts of marriage with reason why it is not registered on time.
-Unregistered copy of Marriage Certificate or Marriage certificate issued by the Church/ or issued by the solemnizing Officer

C.3.Reconstruction
(in case no copy of the original marriage certificate either the owner, the church or the solemnizing officer)
-Negative Certification from the National Statistics Office
-Negative certification issued by the Local civil Registrar
-Affidavit of Affiant attesting the facts of marriage with  reasons why it is not registered on time
-Joint Affidavit
-Birth Certificates of Children containing the date and place of marriage
-Or any documents containing the date and place of marriage


D.Application for Marriage License
-Pre-Marriage counseling
-Certificate of Live Birth
-Baptismal certificate
-Voter’s registration record

Additional Requirement for the following applicants:
For Foreigners:
-Legal capacity to marry issued by the Embassy or
Consular Office of the Philippines
-Certificate of No Record of Marriage (CENOMAR)
-Certificate of Finality and Divorce Decree

For Widow or Widower:
-Death Certificate of deceased spouse
For Applicant who is married but filed a petition for presumptive death of  spouse:
-Certificate of Finality and Court Decree of Presumptive death


E.Registration of Court Decrees

-5 certified machine copies of court order and certificate of finality duly certified by the clerk of court
-5 certified machine copies of birth certificate issued by LCRO
-SECPA Copy  ( Birth certificate from NSO)
E.2. Change or correction of entries in the Certificate of Live Birth/ Marriage/ Death

-Joint Affidavit of Legitimation
-Community tax certificate  (Cedula) of father and mother
F.2.Acknowledgment

-Affidavit of Acknowledgment executed by the father

F.3.Affidavit to Use the Surname of the Father (AUSF)

G.Petition for Change of First Name under Republic Act. No. 9048

G.1. Clearances:
-Barangay Clearance
-Police Clearance
-NBI Clearance
-Court Clearance
-Employer’s Clearance  ,if employed ( State that petitioner has no pending administrative / criminal / civil case
-Affidavit of Non- Employment, if not employed

G.2.  Supporting for the child’s first name (at least any 2 documents)
-Baptismal certificate
-School records
-Voter’s Record
-SSS records
-Employment Records
-Driver’s License
-Marriage certificate
-Birth certificate of siblings

G.3. Publication for two (2) consecutive weeks in a general circulation Newspaper
-After publication, submit 2 copies of Affidavit of Publisher and newspaper clippings


H.Petition for Correction of the Clerical Error in the Certificate of Live Birth/ Marriage/ Death under republic Act.  No. 9048

H.1. For Errors on the child’s last/middle names, or the father’s first/middle/last names, or the mother’s first/ middle /last names
-Birth Certificate of father/ Mother
-Marriage certificate of Parents
-Birth certificate of siblings
-Voter”s Registration of Father/ Mother
-SSS Record of Father/ Mother
-or any documents that show the correct first/ middle / last names of father/ mother

H.2. For errors on the date & place of marriage of parents in the Certificate of Live Birth (in this case the child’s legitimacy should not be affected)
-Marriage certificate of Parents
-Birth certificate of Other siblings
•Other errors to be corrected will be evaluated by the in-charge or the City Civil Registrar
•Errors on the date of birth, status, nationality, and sex are excluded in the coverage of this law it should be filed at the proper court.
